چکیده

Abstract Background Radiographic damage in rheumatoid arthritis (RA) includes erosions and joint space narrowing (JSN). Different mechanisms may underlie their development. The objective of this study was to evaluate predictors these entities separately. Methods Consecutive early RA patients (symptom duration ≤12 months) from a defined area (Malmö, Sweden) recruited during 1995–2005 were investigated. Radiographs hands feet scored by trained reader according the modified Sharp-van der Heijde score. Fat mass lean distribution measured at baseline using dual energy x-ray absorptiometry. Potential erosion JSN progression inclusion 5-year follow-up evaluated. Results Two hundred thirty-three included. 5 years available for 162 patients. median (interquartile) scores 4 (0–8) 8 (1–16), respectively. Rheumatoid factor (RF) robust significant predictor both score progression. In adjusted analyses, anti-CCP antibodies predicted while erythrocyte sedimentation rate predictive outcomes. Smoking high disease activity (DAS28 > 5.1) erosions. Baseline associated with progression, only JSN. Overweight/obesity (BMI ≥ 25 kg/m 2 ) negative ( β = − 0.14, p 0.018, RF, age, score) also when additionally adjusting ever smoking 0.041). Among female patients, effect observed those estimated post-menopausal age (> 51 years), but not younger women. truncal peripheral fat ratio less women, men. Conclusions Overweight had independent status. This seen particular among older women (mainly post-menopausal), younger. Truncal precede BMI influence cartilage might be related hormonal factors.

Predictors of radiographic joint damage in patients with early rheumatoid arthritis.

OBJECTIVE To determine factors at diagnosis, associated with radiographic damage at diagnosis and after one year, in patients with early rheumatoid arthritis (RA). METHODS New patients with early RA were followed up for one year. Possible prognostic factors were duration of complaints, morning stiffness, disease activity score (DAS28), functional status (Health Assessment Questionnaire (HAQ) ...

Age affects joint space narrowing in patients with early active rheumatoid arthritis

BACKGROUND Joint space narrowing (JSN) in rheumatoid arthritis (RA) may be a manifestation of (primary) osteoarthritis becoming more prominent with age. We investigated the severity and predictors of JSN progression among different age groups. METHODS 10-year follow-up data of the BeSt study, a randomised controlled treat-to-target trial in early RA were used. Annual X-rays of hands and feet ...

Existing joint erosions increase the risk of joint space narrowing independently of clinical synovitis in patients with early rheumatoid arthritis

INTRODUCTION Clinical synovitis is often associated with damage to bone and cartilage. Previous data have suggested that joint erosions (JE) are more prevalent than joint space narrowing (JSN) and that the two processes are partly independent of each other. The objective of this study was to evaluate whether the presence of JE in an individual joint can lead to development of JSN and if existin...

Radiographic progression in rheumatoid arthritis.

Radiographic progression is an important outcome measure in clinical trials and observational studies with patients with rheuamtoid arthritis. In this article we describe several aspects of measuring radiographic progression. We introduce the scoring method, discuss scoring methodology and issues regarding reliability of scoring, describe the relation between disease activity, radiographic prog...

Clinical predictors of erosion-free status in rheumatoid arthritis: a prospective cohort study.

OBJECTIVE Treatment algorithms in RA include factors associated with poor prognosis; however, many patients remain erosion free despite years of disease. Our objective was to characterize the group of RA patients without erosions and identify its clinical predictors. METHODS Our study was conducted within a prospective observational cohort of RA patients recruited from the outpatient practice...
